Competitive Grants Application
The Grant Application contains eligibility criteria and application requirements. The application is currently available in MS Word format with a fillable PDF document coming soon.
Please note we are seeking your most up to date financial information. For example, if your organization is applying for the January 12 deadline and your fiscal year ends December 31 please provide the following financial information:
- An operating budget for 1/1/12-12/31/12; and
- An income and expense statement for 1/1/11-12/31/11; and
- An audit for 1/1/10-12/31/10.

If you cannot download the file above, please email grants@communityfoundation.org and we will email the form to you.
Call the Foundation at 413-732-2858 to request a paper application through the mail.
Forms may be reproduced; however, the exact form spacing must be preserved so that there is two inches of blank space at the bottom of the grant application cover page.
Electronically submitted applications (fax or e-mail) will not be accepted.
