- email grants@communityfoundation.org for a Microsoft Word version.
- Call the Foundation at 413-732-2858 to request a paper application through the mail.
Forms may be reproduced; however, the exact form spacing must be preserved so that there is two inches of blank space at the bottom of the grant application cover page.
Electronically submitted applications (fax or e-mail) will not be accepted.
